I actually use two different payroll agencies.  One does managed healthcare
and the other does not.  I give the employee the option of which agency they
would prefer to use based on whether or not they already have healthcare
somewhere else.

The one with managed healthcare is more expensive so raises are slower, but
hey its all a trade off.  The difference is I make all that stuff known up
front.
