I have looked at this for a semi-related project....turned out to be
surprisingly difficult to find a modem that recognised "off hook"
on the
internal side (phones). It was complicated by the fact I also wanted the
modem to do caller ID as well, but the off-hook detection was far more
trouble than I anticipated.

(The project was to integrate telephony into a home automation system - -
we
already had the VR setup, and wanted to be able to dial in and talk to it
remotely - - at some point the "use internal phones too!" was
added).  never
did get to the bottom of that one.
