I guess you could do it with a home-brew PVR and a simple 2-button
remote for channel up/down (actual channel selection is handled by the
PVR). As each channel is selected the new channel logo is displayed as
a splash-screen on the TV for a few seconds, and/or the channel number
is spoken via TTS to give audible feedback.

Still leaves you with the problem of finding a remote with big
buttons, but a DIY-job with a remote-sized case would be a bit easier
for up/down, rather than a numeric keypad.

If you're looking for big number buttons, maybe you could hack one of
those big-button phones - a couple have come up on the Freecycle group
I subscribe to.
