Terminating PSTN and ADSL line to CAT5 patch panel.

I am taking ADSL on one pair, and PSTN on another pair (minus ring
wire), and want to terminate them to the back of my CAT5 patch panel
(IDC punchdowns).

Where do I terminate these pairs to in order that I can connect RJ11
connectors into the RJ45 ports for my ADSL modem and telephone etc
