a/v outlet options

I'm putting an LCD TV on the wall in our kitchen to replace and ageing
TV/VHS combi.

Currently I have a twin back box with a single socket on one side and a
TV coax plate on the other. I'm looking at drilling a cunningly angled
(and rather long) hole through to a side-loft to let me run some other
cables there.

1) I need the plate to have TV-coax + audio(2xphono) s-video(2xphono or
an s-video socket). However, it needs to match MK MCO. What's the best
option? Drill a blank plate and fit what I need?
2) What cable should I run through the wall for max flexibility in the
future? I guess I could run cat5 for a kat5 module but this will also
need 12V from somewhere.
