You can use a Sipura stand-alone if you want to. Asterisk is obviously
a nice-to-have.

You could do this with asterisk + your PC + BT headset, or if you can
find one, you could just connect a Nextlink Bluespoon home connection
kit to an analogue line. This is essentially a handsfree kit for
analogue lines (http://www.expansys.com/product.asp?code=110634)

I haven't seen any hard-phones - either POTS or VOIP - that have a
built in bluetooth interface yet? Would be a very nice find/toy!
