Sounds like a problem with the mixer tap allowing the higher pressure cold
water to flow back up the hot pipe. If you turn the shower to 100% hot,
does it
then get warm?

Could be a non-return valve scaled up or jammed open, possibly even back
pressure cased by a blocked shower head.
