Looks to me like the frame has been double-exposed - this happens when
shooting film and the film hasn't been advanced between the first and
second
shots [either by accident or intention].  If it was shot with a digital
camera, then check that the camera used has a multiple exposure feature -
if
it doesn't then seek help from your local poltergeist removal agency ;)
