Here's my thoughts

Not fussed really whichever is easiest, I think in the same order as played
would make sense and is how it currently works for normal playback.

I would like to see the whole play list perhaps two separate boxes one with
played tracks the one below with to be played.  In normal mode it just
picks up the top track in random it would pick any from the bottom list.

How about move to top (i.e. play next) and bottom (play last). Most of the
time I put a track on and want to here it next rather than wait for the
rest of the playlist and don't want to have to keep pushing the track up
until it eventually climbs to the top.
