RE: Poll of HA kit/ lighting systems

Found a completely free survey tool:

Good and does what I want but the only down side is that it doesn't look
like the results are shareable publically - so results would have to be
published regularly (which I'd rather not have to do).

Anyone know if there's a really good PHP etc survey package that can be
added to a web site that does all this and more?
