No not definite - I have 5 stereo pairs showing up in the
windows/application control panels. 3 definitely work individually as I'v
just checked them. One of the other outputs seems to map on top of the
other though - so maybe there is a limit of 3?

The card is an SB0240 (Audigy 2 Platinum). It was a long time ago that I
played with it and I have swapped cards over since then - From the
supported cards list on the kx site, this is actually a 6.1 card.

This guide shows that it should be possible to get all 4 outputs though:
