Which TV card do you use?
Does MCE cope with all the program guides and stuff when using an external
Sky box?

I use SCART->S-Video input in my TV card to enable it to receive the
signal.

If you buy an MCE remote control, you will get an IR blaster with it, which
will then (once you have gone through the MCE setup) automatically do the
channel changing.

Just about as easy as it gets.
