Just thinking aloud here...

The BT Caller ID signals its arrival by a line polarity reversal just
before
the modem signal and then the first ring... do the US cards have any
ability
to detect this line reversal ? During a call (call waiting) the same line
reversal occurs but obviously there is no ring following.

I think what you are saying is that the modem constantly snoops the 'on
hook' line & the patch detects 'ring' and then looks in an alternate
buffer
for the CID info which should be there now having arrived just before the
first ring ??   I don't think this would then work for the Call Waiting CID
would it ?? The Meteor wont detect this either... or cant we get this
feature ?
