No doubt Andy will have wider experience to draw on, but my personal
experience suggests that it is not possible to have more than 1 phone
per fixed IP address registered with an external service in the way
you describe. The reasons for this are involved, but basically boil
down the fact that the SIP service provider (FWD, etc) will use your
fixed IP address as the key for managing the connection (irrespective
of whether or not you happen to have a STUN aware router).
Running asterisk at each site would indeed solve the problem. You
could also join the sites via VPN and then run a single asterisk box
serving both sites.
