Wandsworth definitely do a quad outlet in a double gang faceplate.
Don't think you can mix RJ45/Coax... with them, however you could use
their custom service if you want to get significantly poorer quickly.

I recall they also do twins in a single gang.
Even the standard stuff is pricey though, I think the quad faceplate
& sockets come to =A380+
