All depends on the area, remember NTL is made up of loads of smaller cable
companies that they brought in the mid to late 90's and never got around to
merging together.

How come you got a direct debit "form" from NTL?
I moved to direct debit some months ago and it was all done over the phone
with no paper involved.
When I queried it I was told that it was their standard practice.
