Wall plate with multiple TV coax

By my OnDigital box I'd like a single-gang sized wall-plate with at
least three coax sockets.

I want this because I need t  have
1) Aerial from room (direct) - connects t  digibox input.
2) Output of digibox t  whole-house splitter at node 0
3) One output of splitter t  feed video/tv

I want t  do this t  avoid having splitters feeding splitters.  I
*could* use a low-loss passive splitter on the digibox output, but I'd
still need two sockets.

What's the best way t  have this wall-plate with multiple coax sockets?

BTW Just bought another 100m reel of CT100 t  finish off this job
(previous reel didn't go as far as planned!) so if anyone needs some
I'll be eager t  sell off what's left over.
