I always use a directory structure of Genre/Artist/Album.  The track
names start 01 - , 02 - etc.
That approach seems to be fairly common.

The playlists I keep in a separate hierarchy, so that I can have
multiple copies.  That way a playlist can appear under multiple genres,
or in other groupings (e.g by year).
