SubInstance names are unique (now).

There are three occasions when outputs are likely to change state at the
same time. In response to a message i.e. direct set, wildcarded message
etc.
As the result of timers expiring that were set at the same time and finally
when I have input to output actions supported and one input sets more than
one output at the same time.
