axc and Asterisk 1.6/Astman 1.1

I'm not sure if anyone else is using this combination - for me PIAF
Bronze and latest axc
I've had to make a number of changes to cope with Astman 1.1 (and I
guess 1.2, but haven't tried that) and I haven't seen them documented
elsehwhere.
The main ones are around dial_callback, where Source becomes Channel,
the callback event for Newcallerid becomes NewCallerid and State is
ChannelStateDesc
Also Link event is deprecated in favour of Bridge and there needs
additional logic since Bridge can be Link or Unlink (in theory and
probably Astman 1.2) but the Unlink event still works in 1.1
