Aside from the error that is now fixed ( there was an issue with the
nickname field), it should work like this
An incoming call comes in and the name is looked up in the db along with
the std. Now SB looks in the images folder first for a jpg file whose
file name matches the number, if it can't find that then it looks for a
jpg with a name that matches the caller's name. Assuming one or other is
found the the cid.lookup message sent out in response to the call will
include the full http:// url to access the image.  The telephone number
should not contain any spaces or dashes and the name should be in the
format displayed in the call log (first name, space, last name).

Creating records based on actual calls works fine btw. If you just want
to add some test ones for now you could do this via access and the
xAP_Contacts table.
