'Shotgun' style twin CAT5 patch lead suppliers ?

Does anyone know if you can get such a thing as 'shotgun' style (i.e.
twin leads joined) pre made CAT 5 patch cables ? Failing that, lengths of
CAT 5 so I can roll my own ?
I've run Component video & audio using baluns across 2 X cat 5 cables
and it's a bit messy using separate leads at the device end, even cable
tied it looks a bit amateurish. It's on my list of things (I've been told )
to tidy up :-)
