you might want to try http://www.sipgate.co.uk (or
Sipgate.com) - these
allow you to have a free number to which if you dial a sipgate to
sipgate number its free... She would, in this case, need a voip phone
(or the pc software version?)

I've also just had a look at the .com site and although you cant
register in the UK for a US number - there is nothing stopping your
sister in law signing up and creating an account for you to add to your
system. She then dials her local number - which in turn your asterisk
system could treat this as a local extension? ( a few leaps of logic -
but it kinda makes sense )
