CPC have a couple of products that might do the trick, see:

for video, and
http://cpc.farnell.com/jsp/endecaSearch/partDetail.jsp?SKU=CS1278701&N=0
for the USB bit.

I'd ear-marked these products for a project that eventually didn't get
funding so I've no personal experience, but they seem to fit the bill.
