I have one and it does what it says.  You do not necessarily have to
sign up
for the monitoring service, but if you want to be notified of an alarm
condition, obviously you do.  Very easy to set up, and there is an X10
interface that you can get for it, but I have yet to explore that route
(sorry Craig ;)).

I bought mine on eBay for £80, except it came with an additional PIR
and fob
(IIRC).
