I use a product called Mailwasher Pro from www.firetrust.com

This polls the server and removes the spam before downloading it, like
all of these programs it takes a short while to learn the good from
the bad but it is definately worth sticking with.

The program does access databases of spam email addresses and adds
those automatically but also gives you control over your own friends,
blacklists and span filters, so you can set up filters which are quite
powerfull.
