My linux box is built around an AOpen AX4SPE Max II, which meets all
those requirements (and some). Haven't had a single problem with it,
other than getting it in the first place - ordered off ebay from the US
in the end. ISTR having seen it listed on a UK website since then
though, so maybe it's now being sold officially over here.

The machine also runs XP occasionally, without any problems (NB, note
the comma :-)

It probably does rather more than you need actually, so you could almost
certainly find a cheaper alternative. Also the 1394 and SP/DIF are off
headers, but they provide brackets.

The intel website has quite a useful list of motherboard that can be
searched by feature(s) IIRC...
