Lead - Vigor VoIP Router to RJ45

I want to put a phone in kitchen using one of the Vigors VoIP ports. 
Rather
than go from the RJ11 - BT Adaptor - RJ45, I can presumably just make up an
RJ11-RJ45 lead so I can put it into the patch panel.

Any idea how this lead would be wired?
