You don't actually *need* a SIP proxy, although it is certainly more
convenient; you can do make do with port forwarding if you have to so
long as the equipment on the other end is NAT friendly. My asterisk
installation plays quite nicely behind a NAT firewall which is
completely SIP / Stun ignorant. So even if you don't have a SIP aware
firewall, VoIP is worth a play if you're so inclined. TelAppliant
(not aware of any others, although they must exist) is now offering
UK PSTN to SIP bridging so you can even accept incoming voip calls on
a "virtual" phone number.
