Mark,

I made my own plates up, these have a combination of speaker sockets, F
sockets etc.

I followed James Hoye's tutorial, see:
http://www.home-automation.info/
then click "Sitting Room" form the LHS menu, in the second para
under the picture click the HOWTO link.

If you use smaller banana sockets you can get 6 pairs onto a double-gang
plate.

Alternatively, I'm pretty sure www.letsautomate.com does them, not sure
about 12-way though.
