Doesn't have IR return, but I use a couple of these guys for feeding my
video around the place...

These are passive, not active, so I doubt if they would be up to the KAT-5
quality level, that being said, the Composite signal from Tivo looks just
fine coming out the far end of a reasonably long stretch of CAT5e and these
guys..

Worth pointing out that the video feed (a BNC plug) being a 75ohm balun at
each end allows SPDIF to traverse some noisy cable without any of the pops
or clicks of cat5 on it's own
