1, I haven't seen flat CAT5, maybe you're thinking of the telephone cable,
which you can get from CPC. (CBMOD4 for 4 core)

If I understand right, you've come across those adaptors which offer
'master
line' or 'secondary' type. The master ones are for phone systems that have
all the extensions wired directly to the incoming BT line (mostly in
commercial type installs), and therefore need a 'master' box for each phone
outlet. If you already have a master box somewhere in the house and
extensions then fed from it, then you'll need 'Secondary' adaptors for the
extensions.

I don't think there is a definite standard for phone-RJ45 as long as it
works in your house, apart from as Mathew said, you can put the 4 cores
onto
the central pins.

I sneaky thing to do is if you are wiring your place for network and phone,
put the phones down 4 cores of a CAT5 and network down the other 4. You can
then split them back out in the wall-boxes, or even do some fancy RJ45
wiring to let you use each socket for either/both.

Still a newbie but I've just done some cat5 cabling of the ground
floor of my house and have a couple of questions: -

1 - where can I buy the flat stranded cat5 cable from, and
2 - how do I connect the BT master socket to the RJ45 sockets? I've
found the adapters for phone to RJ45 (but don't quite understand the
difference between a master line adapter and a secondary one) but
not one that has the BT plug on one ens and an RJ45 on the other (or
am I just being thick trying to do this?)

Any assistance would be much appreciated,
