You can use two single pole relays like this....

+12V_______                ______+12V
\ ___Motor____
0V ________               \______ 0V
Relay A         Relay B

A operated B released moves one way
B operated A released moves the other way

Both operated or both released the motor stops...very quickly because you
activate electronic braking by short circuiting the motor.

If you have +12 and -12V available you can do it like this

+12V_______
\ ________ \____ Motor____ 0V
-12V ______
Relay D   Relay M

Where D = direction and M = Movement
