The tank in the attic provides the positive pressure for the hot water
tank.  The cold water will come straight in from the mains.

As the head pressure is in the ceiling it should be fine

Our hot water tank is on the second floor but the pipes go up into the loft
and back down again so in actual fact the shower unit is above the tank if
only by a few feet.
