The best tool I have for this is a length of 13mm oval conduit. Push a
piece of Cat5e fold in half in the end and it fits tight enough to be taken
along with the conduit as you push in the gaps between floorboards.  I
normally take up the floor board where the cables need going through the
joist that way you end up being able to get more cables through a smaller
hole.

Depends on the depth of the joist but about 25mm diameter is normally
acceptable.
