Your application is probably using system account, and in that case
settings are stored in registry here:

HKEY_USERS\DEFAULT\Software\Microsoft\Internet Explorer\PageSetup\Header
HKEY_USERS\DEFAULT\Software\Microsoft\Internet Explorer\PageSetup\Footer

You need to empty them

This is something that should be done in the application really. I
used some JavaScript to do page printing and the header and footer
are exposed to that as far as I remember, at least with IE they are.
