Combine the two tasks.
1.
Create an electronic copy of the form.
2. Get
a Barcode printing program
3.
Print Barcodes containing all the answers for the form
4. Use
Barcode reader to enter information into form
5.
Print Form
Probably not overly productive but you get to play with the scanner
AND
complete the form :-)

PS. It
might be quicker and have a higher WAF if you complete the form then play
with
the scanner!
Only problem is...SWMBO wanted me to fill in
criminal injuries form (when bottle tattooed my head), but I insisted on
playing with the scanner :-)
