What about a switch on the door? Couple that to the PIR with some logic
like If the switch (door) is closed, & the PIR is on switch the
extractor &
heating on till the door is opened again?

As a safety, you could also have the also program the heating off if the
PIR isn't triggered for 15 minutes (how long are you going to lie in the
bath for :-)
