The alternative, which I've recently been involved with, was to laminate
extra pieces of 4x2 to the original joist, and cut holes through the now
8"
thick "expanded joist".

Of course we did have the benefit of a Ph.D. engineer helping to do the
structural calculations with my wife (who is only an M.Eng.)
